Requirements along with the Registration form
- Original Birth Certificate from the Municipal Corporation and a Xerox copy.
- The Baptismal Certificate (for Catholics and Christians) and the one given by the Municipal Corporation and not of the Hospital.
- Ration card with the child’s name enrolled and a Xerox copy.
- Copy of child’s Aadhaar card.
- Registration fee Rs.500/-
- Paste a family photo and the child’s photo on the form.
- Caste certificate for (SC/ ST/ NT/ DT / OBC /SBC)
- Income certificate of the parents.
